Instead of trying to understand IBS from individual characteristics,  we can take an abstract approach, akin to mathematical abstraction.  Although each patient is different,  they have signs and symptoms in common.<\/p>\n\n\n\n<p class=\"has-background has-very-light-gray-background-color\">Our ambition is to find the  fundamental characteristics that are necessary and sufficient to  describe IBS.  &#8216;<em>Necessary and sufficient<\/em>&#8216; is sometimes expressed as &#8216;<em>if and only if<\/em>&#8216;.  If causes are present an effect will follow <strong>and<\/strong> this effect will only follow if these causes are present.<\/p>\n\n\n\n<p class=\"has-background has-very-light-gray-background-color\">In the field of medicine, we are unlikely to achieve this\nlevel of mathematical precision but we can attempt to find &#8216;<em>necessary and sufficient<\/em>&#8216;\nconditions that best fit most cases of IBS. This will facilitate two\nobjectives: &#8211;<\/p>\n\n\n\n<ol><li>We will be able to identify the essential characteristics of IBS enabling better diagnosis and be able to recognise IBS, even if IBS is a subset of other disorders.<\/li><li>We can isolate signs and symptoms that are not due to IBS and use these to aid diagnosis of the underlying conditions that lead to IBS.<\/li><\/ol>\n\n\n\n<figure class=\"wp-block-image is-resized\"><img loading=\"lazy\" src=\"http:\/\/ibshypo.com\/wp-content\/uploads\/2019\/03\/Fundamental-Characteristics-of-IBS-1024x576.png\" alt=\"\" class=\"wp-image-681\" width=\"1024\" height=\"576\" srcset=\"https:\/\/ibshypo.com\/wp-content\/uploads\/2019\/03\/Fundamental-Characteristics-of-IBS-1024x576.png 1024w, https:\/\/ibshypo.com\/wp-content\/uploads\/2019\/03\/Fundamental-Characteristics-of-IBS-300x169.png 300w, https:\/\/ibshypo.com\/wp-content\/uploads\/2019\/03\/Fundamental-Characteristics-of-IBS-768x432.png 768w, https:\/\/ibshypo.com\/wp-content\/uploads\/2019\/03\/Fundamental-Characteristics-of-IBS-730x410.png 730w, https:\/\/ibshypo.com\/wp-content\/uploads\/2019\/03\/Fundamental-Characteristics-of-IBS.png 1152w\" sizes=\"(max-width: 1024px) 100vw, 1024px\" \/><figcaption><strong>We hypothesize IBS shares fundamental characteristics with organic disorders.  These disorders have signs and symptoms, some common to IBS and some not.  These non-IBS signs and symptoms give a clue to the underlying organic disorder.<\/strong><br><\/figcaption><\/figure>\n\n\n\n<p class=\"has-background has-very-light-gray-background-color\"> IBS signs and symptoms include: &#8211;<\/p>\n\n\n\n<ul><li>Altered bowel habit: constipation, diarrhoea, or both.<\/li><li>Abdominal pain.<\/li><li>Sensation of bloating. <\/li><li>Straining to defecate, producing &#8216;ribbon stools&#8217;.<\/li><li>A feeling of incomplete evacuation.<\/li><li>Relief after a bowel movement.<br><\/li><\/ul>\n\n\n\n<p class=\"has-background has-very-light-gray-background-color\">These signs and symptoms can be derived from two fundamental anomalies in IBS: &#8211;<\/p>\n\n\n\n<ol><li><strong> The gut contracts abnormally.<\/strong><\/li><li><strong>Normal gut stimuli are painful (visceral hyperalgesia). <\/strong><\/li><\/ol>\n\n\n\n<figure class=\"wp-block-image\"><img loading=\"lazy\" width=\"1536\" height=\"864\" src=\"http:\/\/ibshypo.com\/wp-content\/uploads\/2019\/02\/IBS-Fundamentals-e1551174507253.png\" alt=\"\" class=\"wp-image-88\"\/><figcaption> <br><strong>A simplified picture of how abnormal gut contraction and  hyperalgesia  influence the signs and symptoms of IBS.  For example, spasm  (contraction) of the gut and hypersensitivity to pain combine to  produce  a distended abdomen and sensation of bloating.
