Skeletal muscle is attached to bones and used for movement.  Cardiac muscle is present in the heart.  Smooth muscle forms the tissue of hollow internal organs such as blood vessels, the intestines and bladder.  Smooth muscle is involuntary, it is not usually under  conscious  control.  The gut has smooth muscle (see this <a rel=\"noreferrer noopener\" aria-label=\"image (opens in a new tab)\" href=\"https:\/\/www.open.edu\/openlearn\/ocw\/mod\/oucontent\/view.php?id=65373&amp;extra=thumbnailfigure_idp3062160\" target=\"_blank\">image<\/a> from The Open University).<\/p>\n\n\n\n<figure class=\"wp-block-image size-large is-style-default\"><a href=\"https:\/\/www.open.edu\/openlearn\/ocw\/mod\/oucontent\/view.php?id=65373&amp;extra=thumbnailfigure_idm45830435196880\" target=\"_blank\" rel=\"noopener\"><img loading=\"lazy\" width=\"880\" height=\"587\" src=\"https:\/\/ibshypo.com\/wp-content\/uploads\/2021\/03\/image.png\" alt=\"\" class=\"wp-image-5714\" srcset=\"https:\/\/ibshypo.com\/wp-content\/uploads\/2021\/03\/image.png 880w, https:\/\/ibshypo.com\/wp-content\/uploads\/2021\/03\/image-300x200.png 300w, https:\/\/ibshypo.com\/wp-content\/uploads\/2021\/03\/image-768x512.png 768w\" sizes=\"(max-width: 880px) 100vw, 880px\" \/><\/a><figcaption><strong>Above Image from the Open University<\/strong><\/figcaption><\/figure>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\">IBS patients experience excess contractions or &#8216;spasms&#8217;, leading to pain and the sensation of  abdominal  bloating or &#8216;trapped wind&#8217;.&nbsp; These are not true spasms as the onset is gradual.&nbsp; The production of ribbon stools is a consequence of a gut that is contracted and unable to relax, producing long, narrow stools.&nbsp; This would also lead to the need to strain to defecate, since a partially contracted gut has little scope for further contractions to propel its contents.  This is similar to the school bully who offers to shake hands and crushes their victim&#8217;s fingers.  If offered one or two fingers they are unable to exert much pressure. <\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\">As well as a propensity of the gut to contract there is abnormal motility in the colon, the small bowel and the oesophagus of IBS patients.&nbsp; IBS patients have a gut that contracts more readily and a gut that has abnormal motility.  This abnormal motility will be addressed in a later chapter on the Enteric Nervous System.<\/p>\n\n\n\n<p class=\"has-light-green-cyan-background-color has-background\">Patient experience and research confirms there is abnormal smooth muscle contraction throughout the gut of IBS patients. <\/p>\n\n\n\n<hr class=\"wp-block-separator\"\/>\n\n\n\n<h3>Is there  evidence to suggest IBS patients have a more generalized smooth muscle  disorder? <\/h3>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\"><a rel=\"noreferrer noopener\" aria-label=\" (opens in a new tab)\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK526125\/?report=reader\" target=\"_blank\">Smooth muscle is found in the bladder, the vascular system, the uterus, male and female reproductive tracts and the lungs (bronchial smooth muscle).<\/a>  <\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\">IBS patients frequently suffer from &#8216;<a href=\"https:\/\/scihub.wikicn.top\/10.1111\/j.1471-0528.1997.tb11013.x\" target=\"_blank\" rel=\"noreferrer noopener\">irritable bladder<\/a>&#8216;.<\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\">A common symptom of inappropriate contraction \/ relaxation of  vascular smooth muscle is headache, IBS patients have a considerably higher incidence of tension <a rel=\"noreferrer noopener\" aria-label=\"headache (opens in a new tab)\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC1817959\/\" target=\"_blank\">headache<\/a> and <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC5585926\/\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\"migraine (opens in a new tab)\">migraine<\/a>.<\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\">Abnormal contraction of bronchial smooth muscle leads to reduced lung capacity and symptoms of asthma.&nbsp; Compared to the general population, IBS patients have a higher prevalence of <a rel=\"noreferrer noopener\" aria-label=\"respiratory symptoms (33.8% v   5.8%) and asthma (15.8% v 1.45%) (opens in a new tab)\" href=\"https:\/\/www.ncbi.nlm.nih.gov\/pubmed\/11374691?dopt=Abstract\" target=\"_blank\">respiratory symptoms (33.8% v   5.8%) and asthma (15.8% v 1.45%)<\/a>.&nbsp; Note that a propensity to abnormal smooth muscle contraction is not the underlying cause of asthma, rather a contributory factor.<br><\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\">Dysmenorrhoea &#8211;  painful menstruation typically involving abdominal cramps and dyspareunia (painful sexual intercourse) are both more common in IBS patients.<br><\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\"> Thus, there is <a rel=\"noreferrer noopener\" aria-label=\"circumstantial evidence (opens in a new tab)\" href=\"https:\/\/gut.bmj.com\/content\/gutjnl\/27\/1\/37.full.pdf\" target=\"_blank\">evidence<\/a> that:<\/p>\n\n\n\n<p class=\"has-light-green-cyan-background-color has-background\"><strong>IBS patients have a generalized smooth muscle disorder.<\/strong><\/p>\n\n\n\n<p class=\"has-very-light-gray-background-color has-background\">Next we look at <a href=\"http:\/\/ibshypo.com\/index.php\/abnormal-pain-response-2\/\">Abnormal Pain Response<\/a>.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>There are three types of muscle.
